Configuration Sql Server and ODKAggregate

Hi, I need to work with OdkAggregate and SQLServer but i don't understand how is the correct form for set up my ODKAgregate.
Until now I have not been able to access the aggregate from my browser.

I use ODKAggregate v 1.4.15 with SQLServer 2012, but the SQLServer2012 is in another server.
