Converting xlsform to an xform with external select

whenever i convert my xlsform which has external choices to an xform online
the 'xiframe link' it gives me an error an i dont know what the problem is.
Please assist.
I have uploaded the raw xls form.

Amisi,

I just tried this form and it had some warnings (which are fine!) but
no errors. If you are getting errors, please describe them in detail.
If it's just warnings, then you are fine.

it is fine but whenever i copy the csv file and the xml file into odk
collect it gives me an error that the itemsets.csv is missing

Amisi,

You are likely putting the itemsets in the wrong place. They should be
in the [form-filename]-media folder on the device.
