i need help creating an xform

Welcome to ODK! Please read through some of the many excellent tutorials on writing ODK forms, eg Intro to Forms in ODK, and have a go yourself with one of the equally-excellent form builder tools (XLSForm, ODK Build, KoboToolbox, ...).
Post back to the Forum when you have a specific form problem that you cannot otherwise figure out yourself, and I'm sure someone here will be able to help.
