Problem multiple questions in a single screen

Hello,

I'm trying to create a form where multiple prompts questions appear on a
single screen. I can make it work when I have two questions and I followed
the example from the ODK website, but when I change the questions and add
more than two the form no crashes when I load it to my phone. I have
uploaded my file to see if you can help me troubleshoot this problem.

When encountering a problem, use the ODK Validate tool (
http://code.google.com/p/opendatakit/downloads/list ) to ensure the form is
well-formed.

In this case, it gives the all-too-common:

You failed to specify values for your select1 choices.
